Denver Followup Visit Level 1 Retainer

A retainer fee is required to confirm your followup visit.

The fee does not cover the full cost of the visit. Most cases will have a balance due when the case ends and you receive your report. This fee pays in advance a portion of your fee.

A Level I followup visit is a low complexity visit for an established client, with at least one visit in the last 365 days, and in full compliance of all obligations placed by an employer or the FAA.

My payments processor, Square, charges the vendor (me) credit card transaction fees even on refunds/cancellations. Starting on April 11, 2023, cancellation for a self-scheduled appointment is subject to a return fee of (2.9% x retainer) + $0.30. On the same effective date, a cancellation of an invoiced retainer fee is subject to a return fee of (3.3% x retainer) + $ 0.30.

The fee is fully refundable is your visit is canceled within 72 hours of the day and time of your visit. You can change your appointment, but you cannot get a refund, within 24 hours of the day and time of your visit; your rescheduled visit must be within thirty days of your original day and time unless I do not have an opening in the next thirty days. If I don’t have an opening within thirty days, you must select from one of the first three posted openings I share with you.
