You can bring a credit card, cash, or check to the office.

You can make a mobile credit card payment by clicking the image.

Venmo Payments

You can make a Venmo payment. For Venmo, you will need to enter the amount as $1440.

PayPal

You can make a PayPal payment. For Paypal, you will need to enter the amount as $1440.

Where is Dr. Kirk?

Please give yourself four hours for the appointment. Most of the time, appointments end after about three hours, but allowing for a longer interval makes sure that you do not need to return later to finish the assessment.

Dr. Kirk’s office is located at 222 S Main Street, 5th Floor, Salt Lake City, UT 84101. The building is a large office building so I recommend that you arrive AT LEAST ten minutes before your appointment to find me.

Bring a government issued photo ID to your appointment.

The easiest way to find me from the airport is by using a rideshare service or taking the Green Line/West Valley Utah Transit Authority Light Rail.

Light Rail from the Airport

If you are taking the Green Line/West Valley Utah Transit Authority Light Rail Train, take the train to Gallivan Plaza. The public transit runs in a protected median along Main Street. My building will be on the WEST side of Main Street.

The Parking Garage at 222 S Main Street

If you drive and use the parking garage at 222 S Main Street, the easiest way to find the parking garage is to navigate to the Kimpton Hotel Monaco Salt Lake City, 15 W 200 S, Salt Lake City, UT 84101.

Do not turn into the hotel check-in lane.

Instead, drive a few feet forward and then you will see the entrance to the parking lot curving ahead to the left as seen in this picture.

Pay close attention to the short-term parking (less than two hours), reserved monthly parking, delivery-only slots, and oversized vehicle parking. You can be ticketed, booted, or towed if you park in a restricted area.

Short-term parking is 2 hours or less. Do not park in short term parking. Your appointment will take longer than 2 hours.

You pay the parking fee on exiting the parking structure after your appointment. We do not have validated parking for customers to 222 S Main Street.

Once you get inside:

If you arrive during a regular workday Monday through Friday, please check in at the security desk. Tell the reception and security staff that you have an appointment on the fifth floor with Dr. Kirk. You will need a photo ID to get a visitor pass to the fifth floor.

After you swipe your visitor pass, the elevators are straight ahead. The office suites occupy the entire fifth floor so find any receptionist and announce that you have an appointment with Dr. Kirk.

If you arrive on the weekend or a holiday, building reception services will be limited. Please have a seat in the public lobby on the ground floor and send Dr. Kirk a text at (720) 334-8762 after you take a seat in the lobby.

It will also help if you send a selfie with your text so I know who I am looking for. If I’m still with a client I won’t be able to answer your message but I will see your message. I will come and find you and escort you upstairs when it is time for your visit.
